Enduring Power of Attorney - Vic (Medical)
A very simple document that requires the Grantor (the person signing the Power of Attorney) to nominate another person (the Attorney) to be their Attorney to make decisions as to medical treatment only (not financial decisons). The Power is to take effect either from that moment on or for any time that the Grantor is unable to make decisions for themselves.
You will need to know the full name and address of the Grantor and the Attorney.
This Power of Attorney is very short and simply refers to the Attorney having all the rights and powers that the Grantor has in respect to medical treatment decisons only.
This Power of Attorney is valid for Victorian residents only.
